Functional Medicine takes time to change habits and heal the body. Horizon Functional Medicine is a membership based practice so patients commit to a 12 month membership to work with Dr. Olderog and improve their health.

Health Restoration Membership

The Health Restoration membership is designed for patients that had imbalances identified on their functional assessment and want a guided approach to healing. This membership offers not just visits with Dr. Olderog, but also with Kelli Robinson a functional nutrition coach. Patients have appointments with either Dr. Olderog or Kelli about every 6 weeks to maintain accountability and adjust the functional prescription. Functional labs may be ordered to further identify the root cause of the imbalances like gut health testing, hormone testing, heavy metal testing, Lyme and other infection testing etc. A lumen device is utilized to improve metabolic health and Inbody scans monitor body composition and progress. This membership is $249/month for 12 months.

Health Optimization Membership

The Heath Optimization Membership is for patients who do not need as intensive of an approach to optimizing their health but are looking to optimize their health with a functional approach. This Membership includes four 60 minute visits with Dr. Olderog, Inbody scans at each visit, 10% supplement discount and educational offerings. Lumen can be added on to this membership for an additional $25/month. This membership is $149/month for 12 months.

Health Maintenance Membership

The Health Maintenance Membership is for patients who have optimized their health and are looking to maintain their health. This includes two 60 minute visits with Dr. Olderog, Inbody scans at each visit, 10% supplement discount and educational offerings. Lumen can be added on to this membership for an additional $25/month. This membership is $75/month for 12 months.
